Application for Assistance from a Fellowship Fund

To enable the Committee to consider your application, please complete this form and return it to the General Secretary at the above address. Use extra paper if necessary.

1. Name and address of applicant.

2. For what purpose do you need financial help? (Please give a detailed description; see also Question 9).

3. What is the estimated total cost? (Give details).

4. What other bodies have you approached for help?

5. How much have you received from them, or been promised?

6. What sponsoring body or person can we approach for a reference in support of your application? (Please give two if possible.)

7. In certain cases the Fellowship may wish to interview applicants. Please give some indication of when you could be available.

8. In the case of applications from corporate bodies, and when the amount requested is substantial, the Fellowship may wish to see recent Accounts and ask for further information about current fund-raising activities. Please say whom we should approach for this purpose.

9. The object of the Fellowship is to promote understanding and contacts between Eastern Orthodox and Western Christians. Explain how you think your project would further this cause.
